BIRTH AND DEATH REGISTRATION
Births :
For births occurring before JANUARY 26, 2009 only, send completed Statements of Live Birth to the Municipal Clerk's Office - Town of Cobourg, 55 King Street West, Cobourg, ON K9A 2M2. There is no fee to register the birth of a child in Cobourg and all births are to be registered within 30 days. Failure to do so could result in difficulties obtaining a birth certificate. For information on completing the Statement of Live birth, please contact the Municipal Clerk's Office (905)372-4301.
For births occuring after JANUARY 26, 2009, send completed Statements of Live Birth to the Registrar General at PO Box 4800, 189 Red River Road, Thunder Bay, ON P7B 6L8, 1-800-461-2156.
Birth Certificate Applications are available from the above address or in a printer friendly "PDF" format with the link below. All applications must be forwarded directly to the Office of the Registrar General in Thunder Bay (address provided on the reverse of the form). The Ministry of Consumer and Business Affairs has now imposed stricter guidelines on those applying for birth certificates. In order to obtain a birth certificate, applicants are now required to provide the signature of a guarantor. New revised birth certificate application forms are now available at all municipal offices. Section # 5 of the form must be filled out by a guarantor. There are very specific instructions and provisions about who is eligible to act as a guarantor. These provisions are included on the reverse side of the form.

PUBLIC NOTICE
The Ontario Division Registrar's office issued a memorandum dated February 1, 2005 regarding processing times for registration of Birth, Death and Marriage information annoucing that current processing times for vital statistical information may now take up to 10 weeks.
All inquiries regarding vital statistics information processing should be referred to the Registration Division Office of the Registrar General, Ministry of Consumer and Business Services in Thunder Bay:
